JFCS offers a variety of programs tailor-made to assist
seniors in maintaining their independence, self-sufficiency and wellness.
These programs, available at nominal fees and open to seniors of all races and religions, reflect the JFCS commitment
to affordably help the elderly within our community to retain their dignity and to improve their quality of life
as they grow older.
Geriatric Care Management Services
If your aging loved one seems to be more frail, forgets
to take medication, is not eating properly, or is disabled, it may be time to avail yourself of our Care Management
Services.
A JFCS Geriatric Care Manager serves as an advocate for the senior. Once assigned, the GCM Manager will assess
the senior's areas of need and concerns. The GCM will then develop and coordinate an action plan, which may be
implemented by any combination of family members, friends, or outside resources.
Such a plan can include helping with personal affairs, such as the Medicare or Long Term Care entry application
process, finding the right healthcare surrogate, ensuring proper durable powers of attorney, coordinating meal
delivery services, managing the home and more.
Families living at a distance from the Jacksonville area find enormous comfort and peace of mind knowing that their
elderly loved one is under the watchful eye of a JFCS Care Manager.
Senior Transportation -
JFCS arranges transportation for older adults of
the Jewish faith
to religious services, medical appointments, shopping trips and recreational activities for a very affordable fee.
Community Kosher Nutrition Program
The Kosher Nutrition program provides hot, nutritious
kosher meals to the older adults in our community. The program, offered at a convenient location off the JFCS premises,
provides a friendly, social environment in which to enjoy delicious meals and the camaraderie of fellow seniors.
Additional Services
In addition to these specialized programs, JFCS offers
individual counseling by therapists trained in geriatric counseling to address issues that particularly affect
the elderly, such as loss of a spouse, aging, retirement, sexuality, depression and anxiety.
For the children or caregivers of elderly relatives or friends, our Age Spot support group provides a professional
counselor-led forum to share experiences and issues with peers.
